How Medicare IRMAA works in 2026

IRMAA (Income-Related Monthly Adjustment Amount) is a surcharge added to your Medicare Part B and Part D premiums if your income is above a set threshold. It starts at $109,000 in modified adjusted gross income (MAGI) for single filers and $218,000 for married couples filing jointly.

Social Security uses your tax return from two years prior — so your 2026 surcharge is based on your 2024 income. If a life-changing event (like retirement) reduced your income since then, you can appeal using Form SSA-44.
